Why this applies to Dabrafenib

Dabrafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the situation with quercetin and your dabrafenib (Tafinlar). Dabrafenib is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that the lab and small human studies point in both directions: in some cases quercetin seems to slow CYP3A4 (which could raise dabrafenib levels and side effects like fever, joint pain, headache, or skin changes), and in at least one study it sped the enzyme up (which could lower levels and make a cancer medicine work less well).

Honestly, this is mostly theory and weak data, not a proven problem. Food amounts of quercetin are very unlikely to matter. But because dabrafenib is a serious cancer drug, please run any concentrated quercetin supplement past your oncologist or pharmacist first.

Dabrafenib also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the idea behind this pairing. Your body uses an enzyme called CYP2C8 to help clear dabrafenib (Tafinlar). In lab dish studies, quercetin can slow that same enzyme down. In theory, if that enzyme works more slowly, dabrafenib could build up a bit and you might notice more of its usual side effects, things like fever, joint or muscle aches, headache, skin rashes, or feeling unusually tired.

I want to be honest about how soft this evidence is. It comes only from test-tube work, not from people taking these two together, and a human study of another similar drug showed no real effect. So this is more of a theoretical caution than a proven problem. Food amounts of quercetin are very unlikely to matter, but a concentrated supplement is worth a quick chat with your oncology team or pharmacist before adding it.

What Is Dabrafenib Used For?

Brand name: Tafinlar

Dabrafenib is used to treat cancers that carry a specific change (mutation) in the BRAF gene — most often called a BRAF V600E or V600K mutation. These mutations cause cancer cells to grow out of control. Before starting this medicine, a tumor test must confirm the mutation is present.

It can be used alone or — more commonly — paired with another targeted drug, trametinib. Conditions it treats include advanced or inoperable melanoma (skin cancer), certain lung cancers, anaplastic thyroid cancer, low-grade brain tumors (glioma) in children, and other solid tumors with the same mutation that haven't responded to prior treatment.
