Memantine, Donepezil falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Namzaric combines two Alzheimer's medications: donepezil and memantine. Donepezil is partly cleared by a liver enzyme called CYP2D6. In test-tube studies, quercetin can slow this enzyme down. If that happened in your body, donepezil could build up a bit higher than usual, which might mean more of its typical side effects, things like nausea, diarrhea, loss of appetite, trouble sleeping, dizziness, or a slower heart rate.

Here's the important part: this is only lab data, and it has not been shown in real people. The amount of quercetin you get from foods like onions, apples, and tea is very unlikely to matter. If you take a high-dose quercetin supplement, it's worth a quick chat with your pharmacist or doctor first.

Memantine, Donepezil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Namzaric combines two Alzheimer's medicines: memantine and donepezil. The donepezil part is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is the evidence is mixed. In some lab and small studies quercetin seems to slow CYP3A4 (which could let donepezil build up a bit), but in at least one human study it actually sped the enzyme up. So we honestly can't say for sure which way it would go.

If donepezil levels did climb, you might notice more of its usual effects like nausea, diarrhea, dizziness, or a slower heart rate. Most of this is theoretical and food amounts of quercetin are unlikely to matter. Before adding a high-dose supplement, please check with your pharmacist or doctor.
