CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Midazolam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's something a little surprising about this pair. Your body clears midazolam (Versed) using a liver enzyme called CYP3A4. Quercetin can block that enzyme in lab tests, which would normally make a drug build up. But the one real human study we have actually points the other way: quercetin sped up midazolam's breakdown and lowered its blood levels by roughly 24% in some people. In plain terms, that could mean midazolam works a bit less well, so the calming, drowsy, anti-anxiety effect before a procedure might be slightly weaker.
The evidence here is thin and mixed, so I wouldn't lose sleep over it. Midazolam is also usually given in a controlled setting where staff adjust the dose to how you respond. Still, if you take high-dose quercetin supplements, mention it to your anesthesia team or pharmacist beforehand.

Here's the gist for midazolam (Versed): your liver clears this sedative using an enzyme called CYP3A4. In a small study of healthy volunteers, quercetin supplements seemed to speed that enzyme up a bit, lowering midazolam levels in the blood by roughly a quarter. In plain terms, the medicine could be cleared a little faster, so the calming, drowsy, memory-blocking effect might be slightly weaker than expected.
I want to keep this in perspective. The evidence is limited and partly theoretical, and a 24% dip is modest. Food amounts of quercetin almost certainly won't matter; concentrated supplements are the question mark. Since midazolam is usually given by a clinician for procedures or anesthesia, just tell your anesthesia team or pharmacist about any quercetin supplements beforehand.

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. What Is Midazolam Used For?
Brand names: Versed, Seizalam, Nayzilam
Midazolam is used in several different forms depending on the setting. Injectable midazolam (given by IV or IM shot) is used before and during surgery or procedures — such as endoscopy, cardiac catheterization, or bronchoscopy — to cause sedation, reduce anxiety, and prevent memory of the event. It can also be used to start general anesthesia or to keep heavily sedated, ventilated patients comfortable in a critical care unit. Midazolam HCl oral syrup is used specifically in children for sedation and anxiety relief before procedures or anesthesia induction, in a monitored medical setting only.
Nayzilam nasal spray is indicated for a different purpose: the acute treatment of seizure clusters (also called acute repetitive seizures) in people aged 12 and older who have epilepsy. It is used intermittently — not as a daily seizure medicine — when a cluster of seizures breaks through the person's usual pattern.
