Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the thing with Polivy: it's an antibody-drug conjugate, which means it carries a powerful cancer-killing payload (called MMAE) into the lymphoma cells. That payload is partly broken down by the CYP3A4 enzyme. Quercetin can nudge this same enzyme, mostly slowing it down in lab and small human studies, though one study oddly showed the opposite. If quercetin slows the breakdown, slightly more of that toxic payload could linger, which in theory might worsen Polivy's usual side effects like nerve tingling or numbness (peripheral neuropathy), low blood counts, infections, or fatigue.

Honestly, this is mostly theoretical and based on lab work, not real Polivy data. Still, since you're dealing with cancer treatment, please run any quercetin supplement past your oncology team first.

What Is Polatuzumab vedotin-piiq Injection Used For?

Brand name: Polivy

  • Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
