ANTIHYPERTENSIVE DRUGS

Treprostinil falls under ANTIHYPERTENSIVE DRUGS — the group Quercetin interacts with.
Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Treprostinil (Orenitram) is used to treat pulmonary arterial hypertension, and one of the things it does is relax and widen blood vessels, which can lower your blood pressure. Quercetin, in concentrated supplement form, may also nudge blood pressure down a bit. Put them together and those effects could stack up.
What that might feel like is more lightheadedness, dizziness, flushing, headache, or feeling faint when you stand up, which are already common with treprostinil. The good news is this is mostly theoretical and based on weaker evidence, and food amounts of quercetin (apples, onions, tea) are unlikely to matter. Still, before starting a high-dose supplement, check with your pharmacist or doctor so they can keep an eye on your blood pressure.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES
2 of 2Treprostinil also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.
In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the basics: oral treprostinil (Orenitram) is partly cleared by a liver enzyme called CYP2C8. Quercetin can block that enzyme, at least in lab dishes. In theory, slowing that breakdown could let more treprostinil hang around in your body, which might nudge up its usual side effects, things like headache, flushing, jaw pain when you chew, nausea, diarrhea, and low blood pressure that can leave you dizzy.
I want to be honest about how soft this evidence is. It comes from test-tube work, not real people, and a human study with another CYP2C8 drug showed no effect. Food amounts of quercetin almost certainly don't matter; concentrated supplements are the question mark. Before adding a quercetin supplement, please run it by your pharmacist or doctor first.

While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. About Treprostinil
What Is Treprostinil Used For?
Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.
Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
