Valdecoxib falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with Valdecoxib (Bextra), an anti-inflammatory pain reliever in the COX-2 family. Your body partly clears this kind of drug using a kidney transporter called OAT3. In lab dishes, quercetin blocks that transporter pretty strongly. The theory is that if quercetin slows this exit route, more of the drug could hang around, which might mean stronger effects and a higher chance of side effects like stomach upset, fluid retention, raised blood pressure, or kidney strain.
That said, this is only test-tube evidence and has never actually been reported in people, so I wouldn't lose sleep over it, especially with quercetin from food. High-dose supplements are where caution makes more sense. Please check with your pharmacist or doctor before combining them.

Here's the gist with Valdecoxib (Bextra), an anti-inflammatory COX-2 drug. In lab testing, quercetin can block a kidney transporter called OAT1 that helps move certain drugs out of your body. If that transporter is slowed down, the theory is that a drug relying on it could hang around a bit longer, which might mean slightly stronger effects and a higher chance of side effects like stomach upset, fluid retention, raised blood pressure, or kidney strain.
I want to be honest with you though: this is only based on test-tube and animal data, and it has never actually been reported in people. Food amounts of quercetin are very unlikely to matter. If you're considering high-dose supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: your body uses a liver enzyme called CYP2C9 to clear Valdecoxib (Bextra), an anti-inflammatory pain reliever. Quercetin, in concentrated supplement doses, appears to slow that same enzyme down. When the cleanup crew works slower, Valdecoxib can build up a bit higher and stick around longer than usual.
For Valdecoxib, that could mean more of its typical effects, things like stomach upset, fluid retention, raised blood pressure, or kidney strain. Keep in mind this is mostly theoretical, based on lab work and a small study using a related drug, not Valdecoxib itself. Food amounts of quercetin (from apples, onions, berries) are very unlikely to matter. Before pairing high-dose supplements with this medicine, check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Most lab work suggests quercetin slows CYP3A4, which could let a bit more Valdecoxib build up, possibly meaning more typical NSAID side effects like stomach upset, fluid retention, or higher blood pressure. One human study actually hinted the opposite, so the direction isn't fully settled.
