Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the short version for anyone taking doravirine (Pifeltro) alongside resveratrol. Your body clears doravirine using a liver enzyme called CYP3A4. In test-tube studies, resveratrol can slow that enzyme down a bit. If that happened in real life, doravirine levels could creep up, which might mean more of its usual side effects like headache, nausea, dizziness, or trouble sleeping.

The good news is that this is mostly a theoretical concern based on lab work, not strong human data. In fact, one human study using a hefty 3,000 mg of resveratrol daily didn't require dose changes for CYP3A4 drugs. Doravirine is not a prodrug, so it still works normally. Still, since keeping HIV controlled matters, run any resveratrol supplement past your pharmacist or doctor first.

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used together with other antiretroviral medications to treat HIV-1 infection in adults and in pediatric patients weighing at least 35 kg (about 77 lbs). It is approved for people who have never been on HIV treatment before, as well as for people who are already virologically suppressed — meaning their HIV is currently undetectable — and want to switch to a new stable regimen.

Pifeltro is not used alone; it must always be part of a combination HIV regimen. Patients switching to a Pifeltro-based regimen must have no history of treatment failure and no known resistance to doravirine.
