Elbasvir, Grazoprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Your hepatitis C combo, Zepatier (elbasvir and grazoprevir), gets processed in part by a liver enzyme called CYP3A4. In test-tube studies, resveratrol can slow that enzyme down a bit. The thinking is that if it slows the breakdown, more of the drug could linger in your body, which might mean a higher chance of side effects like tiredness, headache, nausea, or changes in liver-related lab values.

Here's the reassuring part: this is mostly a theoretical concern from lab work, not real patients. In one human study, even high doses of resveratrol (3,000 mg daily) didn't require any dose changes for CYP3A4 medicines. Still, because you're treating an active infection, it's worth a quick check with your pharmacist or doctor before adding resveratrol.

What Is Elbasvir and Grazoprevir Used For?

Brand name: Zepatier

Zepatier is used to treat chronic hepatitis C virus (HCV) genotype 1 or genotype 4 infection in adults and children 12 years of age and older (or weighing at least 30 kg). It works against both the most common strain of HCV (genotype 1) and genotype 4.

Depending on the specific genotype, prior treatment history, and certain viral characteristics, Zepatier may be prescribed alone or combined with another antiviral medicine called ribavirin. Your prescriber will determine the right regimen and duration for your situation.
