Lurbinectedin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down lurbinectedin (Zepzelca). In test-tube studies, resveratrol can slow that enzyme down. If that happened in real life, lurbinectedin could clear out of your body more slowly and build up, which in theory might mean stronger effects and more side effects like low blood counts, fatigue, nausea, or a higher infection risk.

The reassuring part is that the evidence here is weak. This concern comes from lab studies, and human research found that even 3000 mg of resveratrol daily didn't require dose changes for CYP3A4 drugs. Still, lurbinectedin is potent chemotherapy, so please run any resveratrol supplement past your oncology team before adding it.

What Is Lurbinectedin Injection Used For?

Brand name: Zepzelca

Zepzelca (lurbinectedin) injection is approved for adults with small cell lung cancer (SCLC) in two settings. First, Zepzelca is used in combination with atezolizumab (or atezolizumab and hyaluronidase-tqjs) as maintenance treatment for extensive-stage small cell lung cancer (ES-SCLC) in patients whose disease did not progress after initial treatment with those same immunotherapy drugs plus carboplatin and etoposide.

Second, Zepzelca is approved as a single agent for adults with metastatic small cell lung cancer whose cancer has worsened during or after platinum-based chemotherapy. This second use was granted accelerated approval based on tumor response rates, and continued approval depends on results from confirmatory clinical trials.
