Nelfinavir Mesylate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Resveratrol interacts with.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses a liver enzyme called CYP2C19 to help break down nelfinavir (Viracept). In test-tube studies, resveratrol can slow that enzyme down. If that happened in a real person, nelfinavir could clear a little more slowly and build up, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset. Nelfinavir is an active drug, not a prodrug, so blocking this enzyme would tend to raise its levels, not weaken it.
The honest part: this is only lab-dish evidence and has never been reported in actual patients. With an HIV medicine, though, keeping levels steady matters, so please run any resveratrol supplement past your pharmacist or HIV provider first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the short version: your HIV medicine nelfinavir (Viracept) is cleared from your body partly by a liver enzyme called CYP3A4. In test-tube studies, resveratrol can slow that enzyme down. In theory, that could let nelfinavir levels creep up, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset. Nelfinavir is an active drug (not a prodrug), so blocking the enzyme would tend to raise its effect, not lower it.
The reassuring part is that this is mostly lab-based. One human study using a hefty 3000 mg of resveratrol daily for 8 weeks didn't require any dose changes for CYP3A4 medicines. Still, because keeping HIV controlled matters, run this by your pharmacist or doctor before adding resveratrol.
