Tofacitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Resveratrol interacts with.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with resveratrol and tofacitinib (Xeljanz). In lab studies, resveratrol can block an enzyme called CYP2C19, which is one of the pathways your body uses to clear tofacitinib. If that pathway slows down, tofacitinib could build up a bit and you might notice more of its usual effects, like a higher chance of infections, headaches, or changes in blood counts and cholesterol.
That said, I'd keep this in perspective. This is theoretical and based only on test-tube research, with nothing actually reported in people. Food amounts (grapes, berries) are very unlikely to matter; concentrated supplements are the bigger question. Don't stop or change anything on your own. Just give your pharmacist or doctor a heads-up before adding resveratrol so they can keep an eye on you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Tofacitinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Your body relies partly on an enzyme called CYP3A4 to clear tofacitinib (Xeljanz) from your system. In a test tube, resveratrol has been shown to slow that enzyme down. If that happened in real life, tofacitinib could build up a bit, which in theory might raise the chance of side effects like headaches, increased risk of infections, or changes in your blood counts and cholesterol.
Here's the reassuring part: this is mostly a lab finding, not something proven in people. In fact, a human study using a high dose (3000 mg daily for 8 weeks) found no need to adjust CYP3A4 medications. So the concern is largely theoretical. Still, since tofacitinib affects your immune system, it's worth a quick chat with your pharmacist or doctor before adding resveratrol.

What Is Tofacitinib Used For?

Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
