Voriconazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP2C19 to help break down voriconazole. Resveratrol has been shown in lab dish studies to slow that same enzyme down. In theory, that could let voriconazole build up a bit higher than expected, which might nudge up the risk of its usual side effects, things like blurred or color-changed vision, nausea, headache, or changes in liver tests.

I want to be honest about the strength of this, though. This concern comes only from test tube research, and it has never actually been reported in real people taking the two together. So I wouldn't lose sleep over it, but since voriconazole levels can swing quite a bit between people, it's worth a quick chat with your doctor or pharmacist before adding a resveratrol supplement.

Voriconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the gist with voriconazole (Vfend) and resveratrol. Your body clears voriconazole partly through a liver enzyme called CYP3A4. In lab dish studies, resveratrol can slow that enzyme down. In theory, slowing it could let voriconazole build up a bit more than expected, which might mean more of its usual side effects, like vision changes, nausea, headache, or higher liver enzyme readings.

The reassuring part: this concern comes only from test-tube data, not from people. In an actual human study, fairly high-dose resveratrol (3000 mg a day) did not force any dose changes for CYP3A4 medicines. So I wouldn't lose sleep here. Still, since voriconazole is a serious antifungal often watched with blood levels, just mention any supplement you're taking to your pharmacist or doctor.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
