Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the simple version: your body uses an enzyme called CYP3A4 to help break down voxelotor (Oxbryta). In test-tube studies, resveratrol can slow that enzyme down a bit. In theory, if the enzyme works more slowly, voxelotor could build up a little and you might notice more of its side effects, things like headache, nausea, stomach upset, diarrhea, or feeling tired.

That said, I don't want to overstate this. The concern comes only from lab research, and an actual human study using a high dose of resveratrol (3000 mg daily for 8 weeks) did not require any medication dose changes. So real-world risk looks low. Still, it's worth a quick chat with your pharmacist or doctor before adding resveratrol, just so they can keep an eye on how you feel.

Brand name: Oxbryta

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
