The anthraquinone constituents of rhubarb have been shown to induce nephrotoxicity in animal research. Additionally, in a case report, a 23-year old female presented with kidney failure after taking 6 tablets of a proprietary slimming agent (found to contain the anthraquinones emodin and aloe-emodin from rhubarb) daily for 6 weeks and then adding diclofenac 25 mg 4 times daily for 2 days. The authors postulate that the anthraquinone constituents of rhubarb contributed to the renal dysfunction, and the addition of diclofenac, a nephrotoxic drug, led to renal failure. Until more is known, advise patients to avoid taking rhubarb if they are taking other potentially nephrotoxic drugs.

Here's the gist for Coricidin HBP Flu (which combines acetaminophen, chlorpheniramine, and dextromethorphan). The concern isn't with how your body breaks down these medicines. It's about the kidneys. Rhubarb root contains plant compounds called anthraquinones, and in animal studies and one case report, heavy long-term use seemed to stress the kidneys. Pair that with a drug that can also be hard on the kidneys, and the worry is added strain.
Honestly, the evidence here is thin and theoretical, based mostly on lab and animal data plus a single case involving a different drug. Eating rhubarb stalks in pie won't matter. Concentrated rhubarb root supplements taken daily for weeks are where caution makes sense. If you regularly use rhubarb supplements, just check with your pharmacist before reaching for this combo.

Some animal research suggests that anthraquinones in rhubarb might have hepatotoxic effects. Also, rhubarb use has been linked to at least 24 cases of liver injury, although details on the dose of rhubarb and duration of use in these cases is unclear.

Here's the thing to keep in mind with Coricidin HBP Flu: the acetaminophen part is one of the most common causes of liver stress when too much is taken. The concern with rhubarb root supplements is that some of their natural compounds (anthraquinones like emodin) have shown possible liver-stressing effects in animal studies, and rhubarb has been loosely linked to a small number of liver injury reports. Stacking two things that lean on the liver, at least in theory, could add up.
I want to be honest that this is a theoretical interaction, not something proven in people. Eating rhubarb stalks in pie isn't the worry here, concentrated root supplements are. Watch for yellowing skin, dark urine, belly pain, or unusual tiredness, and check with your pharmacist before combining them.
