Acetaminophen, Codeine, Methocarbamol falls under CNS DEPRESSANTS — the group Sage interacts with.

This combo product already has two ingredients that can make you drowsy: codeine (an opioid) and methocarbamol (a muscle relaxant). The concern here is that some compounds in sage may also have a mild calming, sedative effect on the brain. Stacking these together could, in theory, add up to more sleepiness, dizziness, slowed thinking, or feeling unsteady on your feet.
I want to be honest about the strength of this: it's based only on lab and animal studies, not real patients, so it's theoretical. Sprinkling sage on your dinner is very unlikely to matter. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you take this regularly, just check with your pharmacist or doctor before adding a sage supplement.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the wrinkle with this combo product: one of its ingredients, codeine, is a prodrug. It doesn't do much until your body uses an enzyme called CYP2D6 to convert it into morphine, which is what actually relieves pain. Lab studies hint that sage might block CYP2D6. If that happened in real life, you'd convert less codeine to morphine, so you could get weaker pain relief rather than more side effects.
The good news is this is theoretical, based only on test-tube work, and it has never been reported in people. Sprinkling sage on dinner almost certainly won't matter. Concentrated sage extracts or the essential oil are where I'd be a little more cautious. If you take sage supplements regularly, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

Animal research suggests that drinking common sage (Salvia officinalis) tea increases the expression of CYP2E1. So far, this interaction has not been reported in humans.

Here's the gist for your Robaxacet-type combo (acetaminophen, codeine, and methocarbamol). The piece that touches sage is the acetaminophen. One of the liver pathways that handles acetaminophen is an enzyme called CYP2E1. In animal studies, drinking sage tea seemed to rev up that enzyme. In theory, a faster CYP2E1 could shift more acetaminophen toward a byproduct that is harder on the liver, especially at higher acetaminophen doses or with alcohol.
I want to be honest about the strength here: this is lab and animal data only, and it has never actually been reported in people. A little sage in cooking is very unlikely to matter. Concentrated sage extracts or strong sage tea are where I'd be more cautious. If you use sage regularly, just run it past your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
