The piece of this combo that matters here is the hydrocodone, an opioid painkiller that can make you drowsy and slow your breathing. Sage has some natural compounds that, at least in lab studies, seem to have a mild calming or sedative effect on the brain. The worry is that stacking the two could add up, leaving you more sleepy, foggy, or lightheaded than the medicine alone would.

Honestly, this is a theoretical concern based on test-tube and animal research, not real patient reports, so I wouldn't lose sleep over it. A little sage in your cooking is very unlikely to do anything. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you take both regularly, just run it by your pharmacist or doctor first.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

The piece to watch in this combo product (Damason-P) is the hydrocodone. Hydrocodone is partly handled by a liver enzyme called CYP2D6, which converts some of it into hydromorphone, a stronger pain-relieving form. Lab tests (not real people) suggest sage may block CYP2D6 a little.

So in theory, concentrated sage could slow that conversion and shift hydrocodone's effects, which makes this hard to predict. Honestly, this is only a test-tube finding and hasn't shown up in patients, so I wouldn't lose sleep over it. Cooking with sage is a non-issue. If you're taking sage capsules, extracts, or the essential oil, just mention it to your pharmacist or doctor, especially if you notice extra drowsiness or weaker pain control.

This combo pill pairs aspirin and caffeine with hydrocodone, an opioid pain reliever. The hydrocodone piece is the part to watch here, since it's processed in part by a liver enzyme called CYP3A4. Lab studies suggest concentrated sage extracts may slow that enzyme down a bit. In theory, that could let hydrocodone build up and lead to stronger effects like extra drowsiness, dizziness, constipation, nausea, or slowed breathing.

That said, this is only seen in test-tube research so far, never reported in real people, so the actual risk is likely small. Sprinkling sage on your food almost certainly won't matter; concentrated extracts or sage oil are the bigger question. If you regularly take sage supplements, just give your pharmacist or doctor a heads-up to be safe.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
