Dofetilide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the idea behind this pairing. Lab tests suggest sage may slow down an enzyme called CYP3A4, which is one route your body uses to clear dofetilide. If that enzyme gets blocked, dofetilide could build up to higher levels than expected. With dofetilide, that matters a lot because it's a narrow-margin heart rhythm drug. Too much of it can stretch out the heart's electrical timing (the QT interval) and trigger a dangerous rhythm called torsades de pointes.

The good news: this is only theoretical, based on test-tube work, and hasn't been seen in real people. Cooking with sage is very unlikely to be a problem. Concentrated extracts or sage oil are the bigger question. Before adding any sage supplement, please run it by your doctor or pharmacist first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

About Dofetilide

What Is Dofetilide Used For?

Brand name: Tikosyn

Dofetilide capsules are used to convert atrial fibrillation (AFib) and atrial flutter (abnormal, rapid heart rhythms) back to normal sinus rhythm, and to help keep those rhythms from coming back. This applies to patients who have had AFib or atrial flutter lasting more than one week and who have already been converted to a normal rhythm.

Because dofetilide can cause serious, life-threatening heart rhythm problems, it is reserved for patients whose AFib or atrial flutter causes significant symptoms. It has not been shown to be effective in paroxysmal atrial fibrillation (AFib that starts and stops on its own). Tikosyn is an FDA-approved brand of dofetilide capsules with these same indications.
