Here is the interaction we found between these two, with the evidence behind it.
ANTIHYPERTENSIVE DRUGS
Why this applies to Enalapril Maleate, Hydrochlorothiazide
Enalapril Maleate, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Sage interacts with.
Animal research suggests that common sage (Salvia officinalis) can cause prolonged blood pressure reduction. However, clinical research suggests that Spanish sage (Salvia lavandulaefolia) can increase blood pressure in some people with hypertension. Until more is known, use with caution.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Vaseretic teams up an ACE inhibitor (enalapril) with a water pill (hydrochlorothiazide) to lower your blood pressure. Sage may tug at that number in either direction, so the worry here is mostly about your blood pressure becoming unpredictable. One type of sage seems to lower pressure further in animal studies, which could add to your medication and leave you dizzy, lightheaded, or faint, especially when standing up. Another type may actually nudge pressure up in some people, which could work against your treatment.
Honestly, this is theoretical and comes from lab and animal work, not solid human studies. A little sage in cooking is unlikely to matter. Concentrated extracts or sage oil are where I'd be cautious. Check with your pharmacist before starting a supplement.

About Sage
What is Sage?
- Sore throat and mouth irritation
- Digestive upset
- Excessive sweating
- Memory and thinking
- Menopause symptoms (hot flashes)
- General antioxidant support
Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
About Enalapril and Hydrochlorothiazide
What Is Enalapril and Hydrochlorothiazide Used For?
Brand name: Vaseretic
Enalapril and hydrochlorothiazide tablets — sold under brand names including Vaseretic and Enalapril maleate and hydrochlorothiazide — are used to treat high blood pressure (hypertension). This combination is not meant as the first treatment tried; it's generally used when a single blood pressure medicine hasn't done the job on its own.
Because this is a fixed-dose combination, your doctor will typically adjust each medicine separately first, then switch you to the combination tablet once the right doses are found.
