Dofetilide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with goldthread and your dofetilide (Tikosyn). Goldthread's main active compound, berberine, might gently block a liver enzyme called CYP3A4. If that happens, your body could clear dofetilide a bit more slowly, letting its level creep up. That matters a lot here, because dofetilide is a heart rhythm drug with a very narrow safety window. Even small increases can raise the risk of a dangerous rhythm problem (QT prolongation), with symptoms like dizziness, fainting, or palpitations.

The good news: this is only a theoretical concern based on lab work, not real human reports. Still, because dofetilide is so dose-sensitive, I'd play it safe. Please check with your doctor or pharmacist before adding goldthread.

About Dofetilide

What Is Dofetilide Used For?

Brand name: Tikosyn

Dofetilide capsules are used to convert atrial fibrillation (AFib) and atrial flutter (abnormal, rapid heart rhythms) back to normal sinus rhythm, and to help keep those rhythms from coming back. This applies to patients who have had AFib or atrial flutter lasting more than one week and who have already been converted to a normal rhythm.

Because dofetilide can cause serious, life-threatening heart rhythm problems, it is reserved for patients whose AFib or atrial flutter causes significant symptoms. It has not been shown to be effective in paroxysmal atrial fibrillation (AFib that starts and stops on its own). Tikosyn is an FDA-approved brand of dofetilide capsules with these same indications.
