Fentanyl citrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for fentanyl. Your liver uses an enzyme called CYP3A4 to break fentanyl down and clear it from your body. The berberine in goldthread may slow that enzyme down, at least based on early lab work. If that happens, fentanyl could hang around longer and build up higher than expected. With a strong opioid like fentanyl, that's worth taking seriously, since too much can mean heavy sedation, confusion, very slow or shallow breathing, and in severe cases dangerously depressed breathing.

The honest caveat: this is theoretical, drawn from pharmacology, and hasn't been confirmed in people. Still, because fentanyl leaves so little room for error, I wouldn't combine these on your own. Please check with your pharmacist or doctor first.
