Trabectedin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist: your body clears trabectedin (Yondelis) mainly through a liver enzyme called CYP3A4. In lab tests, water-based sage extracts seem to slow that same enzyme down. If sage blocked CYP3A4 in real life, trabectedin could break down more slowly and build up in your blood, which might mean stronger effects and a higher chance of side effects like nausea, severe fatigue, low blood counts (raising infection and bleeding risk), and liver irritation.

The honest part: this is only a theoretical concern from test-tube data, and it hasn't been seen in actual patients. Cooking with sage is very unlikely to matter; concentrated extracts or capsules are the bigger question mark. Since trabectedin is serious chemo, please run any sage supplement by your oncology team or pharmacist first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. What Is Trabectedin Injection Used For?

Brand name: Yondelis

Yondelis (trabectedin injection) is used to treat adults with liposarcoma or leiomyosarcoma — two types of soft tissue sarcoma (cancer of connective tissue) — when the cancer cannot be removed by surgery (unresectable) or has spread to other parts of the body (metastatic). It is indicated for patients who have already received a prior treatment regimen containing an anthracycline (a type of chemotherapy).
