St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.

Here's the concern with this combination. Trabectedin (Yondelis) is handled by a liver enzyme called CYP3A4, and St. John's wort speeds that enzyme up. The worry is that the herb could clear trabectedin out of your body faster than intended, leaving you with lower drug levels. For a chemotherapy drug fighting a serious cancer like liposarcoma or leiomyosarcoma, that could mean it simply doesn't work as well as it should.

This is rated a major, probable interaction, so it's worth taking seriously even though it isn't a full-strength study behind it. Because trabectedin is given by infusion under cancer-care supervision, please don't add or stop any supplement on your own. Talk with your oncology team or pharmacist first so your treatment stays on track.

What Is Trabectedin Injection Used For?

Brand name: Yondelis

Yondelis (trabectedin injection) is used to treat adults with liposarcoma or leiomyosarcoma — two types of soft tissue sarcoma (cancer of connective tissue) — when the cancer cannot be removed by surgery (unresectable) or has spread to other parts of the body (metastatic). It is indicated for patients who have already received a prior treatment regimen containing an anthracycline (a type of chemotherapy).
