Why this applies to Azathioprine
Azathioprine falls under HEPATOTOXIC DRUGS — the group Saigon Cinnamon interacts with.
Saigon cinnamon contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about pairing Saigon cinnamon with azathioprine: this isn't about one speeding up or slowing down the other. Azathioprine can be tough on the liver on its own, and Saigon cinnamon (a cassia type) is high in coumarin, a compound that has stressed the liver in animal studies and in people taking very large amounts. So the worry is simply two liver burdens stacking up, not a change in how your azathioprine works.
To be honest, this is a theoretical concern based on how these things behave, not strong human proof. A sprinkle of cinnamon on your oatmeal is very unlikely to matter. Regular, concentrated cinnamon supplements are where I'd pump the brakes. Since your team already watches your liver labs on azathioprine, check with your pharmacist or doctor before adding any cinnamon capsules.

Saigon cinnamon is a flavorful spice from a Cinnamomum tree grown mainly in Vietnam, and it is a type of 'cassia' cinnamon. Some people use it for blood sugar and digestion, but the human evidence is limited. Because cassia cinnamons like this one are high in a compound called coumarin, regularly taking large supplement doses may be hard on the liver in sensitive people, so use food amounts and check with your pharmacist before taking concentrated products.

What Is Azathioprine Used For?
Brand names: Imuran, Azamune
Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.
Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
