Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Sorrel interacts with.

Theoretically, sorrel might cause additive effects and side effects when used with anticoagulant or antiplatelet drugs.
In vitro, sorrel has been shown to inhibit platelet aggregation. However, this effect has not been reported in humans.

Here's the quick picture: alteplase (Activase) is a powerful clot-busting drug given in the hospital for things like a heart attack, stroke, or a blood clot in the lungs. Its whole job is to break down clots, so anything that also nudges your blood toward less clotting could, in theory, add to the bleeding risk. In lab dishes, sorrel has slowed platelets (the cells that help blood clot), which is where this concern comes from.

The important part: this effect has only shown up in test-tube studies, never in actual people, so it's theoretical and the evidence is weak. Since alteplase is given by clinicians in a closely monitored setting, this isn't something you'd manage on your own. Still, mention any sorrel supplements to your doctor or pharmacist so they have the full picture.
