Here is the interaction we found between these two, with the evidence behind it.
P-GLYCOPROTEIN SUBSTRATES
Why this applies to Acalabrutinib
Acalabrutinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Strawberry interacts with.
Drugs that might be affected include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Calquence (acalabrutinib). This medicine is moved around and pushed out of your cells by a transporter called P-glycoprotein. In a test-tube study, strawberry extract slowed that transporter down. In theory, blocking it could let a little more acalabrutinib stay in your body, which might mean somewhat stronger effects or more side effects like headaches, bruising or bleeding, fatigue, or a higher infection risk.
The important thing: this is based only on lab research, not on people, so we really don't know if it matters in real life. Eating strawberries as food is very unlikely to cause a problem. A concentrated strawberry supplement is the bigger question mark. To be safe, just run any strawberry supplement past your pharmacist or oncologist before adding it.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Acalabrutinib page
“For anyone taking acalabrutinib (Calquence) for chronic lymphocytic leukemia or lymphoma, the supplement list below deserves genuine scrutiny. Of 144 ingredients on file, nearly all act on CYP3A4, the liver enzyme that clears the drug, or on P-glycoprotein, a transporter that moves it, so they can push its level too high or leave too l…”

- Blood sugar support
- Skin and oral health
- Source of vitamin C and fiber
Strawberry is a popular, nutrient-rich fruit that supplies vitamin C, fiber, and antioxidant plant compounds. About Acalabrutinib
What Is Acalabrutinib Used For?
Brand name: Calquence
Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).
Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
