Aspirin, Codeine Phosphate, Phenobarbital falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP2D6.
Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.

This combination pill includes codeine, which is the piece that matters here. Codeine is a prodrug, meaning it does nothing for pain until your body converts it into morphine using an enzyme called CYP2D6. Syrian Rue's harmala alkaloids may block CYP2D6, so less codeine gets turned into morphine. In plain terms, you could get less pain relief than expected, even though the dose looks the same.

Right now this is only a theoretical concern based on lab studies, not real patient cases, so I don't want to alarm you. Still, Syrian Rue carries its own toxicity risks and acts as an MAO inhibitor, which is a separate worry. Please check with your pharmacist or doctor before pairing the two.
