Hyoscyamine, Phenobarbital falls under ANTICHOLINERGIC DRUGS — the group Syrian Rue interacts with.
Syrian rue contains harmine. Animal research suggests that intravenous administration of harmine potentiates the effects of acetylcholine or increases acetylcholine levels.

Here's the gist for the hyoscyamine part of this combo product. Hyoscyamine works by blocking acetylcholine, which is how it calms stomach cramps and reduces spasms. Syrian Rue contains harmine, and animal studies hint that it may raise acetylcholine levels or boost acetylcholine's activity. If that holds true in people, it could push against what hyoscyamine is trying to do, meaning you might get less relief from your cramping or spasms.
I want to be honest with you: this is theoretical, based mostly on lab and animal work, not strong human data. So I'm not raising alarm bells. That said, Syrian Rue carries real toxicity risks on its own, and phenobarbital is a serious medicine too. Please loop in your pharmacist or doctor before mixing them.

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment.
