Why this applies to Ivosidenib

Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the gist for your leukemia (or bile duct cancer) medicine, ivosidenib. Your body clears this drug using a liver enzyme called CYP3A4. A compound in tangerine, called tangeretin, sped up that enzyme in a lab dish. In theory, that could push your body to break down ivosidenib a little faster, which might lower its levels and make it work less well against the cancer.

The good news: when researchers actually gave people tangerine juice with a similar drug, it didn't meaningfully change how that drug was processed. So real-world risk here looks low, and enjoying a tangerine as food is unlikely to cause trouble. Still, because ivosidenib is a serious cancer treatment, I'd run any concentrated tangerine or citrus supplements past your oncology pharmacist first.

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
