Theacrine and Hydromorphone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CNS DEPRESSANTS

Why this applies to Hydromorphone

Hydromorphone falls under CNS DEPRESSANTS — the group Theacrine interacts with.

Theoretically, theacrine might alter the effects of CNS depressants.
Animal research shows that low doses of theacrine have sedating effects, whereas high doses might have stimulant effects. Depending on the dose of theacrine used, it might increase or decrease the effects of CNS depressants. However, these effects have not yet been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Hydromorphone is a strong opioid pain reliever that calms the nervous system. It can cause drowsiness, slowed breathing, low blood pressure, and confusion. Theacrine is a caffeine-like supplement, and how it might mix with an opioid is honestly a bit of a wild card.

Here's the catch: animal studies hint that low doses of theacrine may act sedating, which could add to hydromorphone's drowsiness, while higher doses may act like a stimulant and partly mask it. Neither effect has actually been shown in people, so this is theoretical. Still, masking an opioid's sedation could lead you to misjudge your dose. Please check with your pharmacist or doctor before pairing these rather than adjusting anything on your own.

From our pharmacist’s overview on the Hydromorphone page

“Hydromorphone (Dilaudid, Palladone) is a potent opioid, and the risk that matters most with any opioid is slowed breathing. Most of this list pushes in exactly that direction.”

About Theacrine

What is Theacrine?

  • Energy and reduced fatigue
  • Focus and mental alertness
  • Mood support
  • Pre-workout performance
  • Caffeine-like stimulation with possibly less tolerance

Theacrine is a caffeine-like compound found naturally in certain tea plants and is sold in supplements for energy, focus, and mood. Early human studies suggest it may give a stimulant-like boost, but the overall evidence is limited and long-term safety is not well known. Talk with your pharmacist or doctor before using it, especially if you are sensitive to stimulants or have heart problems.

About Hydromorphone

What Is Hydromorphone Used For?

Brand names: Dilaudid, Hydromorph Contin, Palladone

Hydromorphone is used to manage pain severe enough to require an opioid analgesic — when non-opioid medicines or other options have not worked or are not expected to work well enough. It is available in several forms: oral tablets, oral solution, rectal suppositories, and injectable formulations given into a vein, muscle, or under the skin.

Hydromorphone hydrochloride extended-release tablets are specifically for severe, persistent pain that requires around-the-clock opioid treatment for an extended period, and are only for opioid-tolerant patients — they are not for as-needed use. The high-potency injectable formulation is also reserved for opioid-tolerant patients who need higher opioid doses.
