Here is the interaction we found between these two, with the evidence behind it.
Oral Drugs
Why this applies to Baloxavir Marboxil
Baloxavir Marboxil falls under Oral Drugs — the group Thyroid Extract interacts with.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the thing to keep in mind with Xofluza (baloxavir marboxil) and a thyroid extract supplement. This interaction is really about your thyroid product, not your flu medicine. Thyroid hormones are absorbed best on an empty stomach, and they can get bound up by minerals and certain other pills taken at the same time, which makes the thyroid dose work less reliably.
The good news is that baloxavir is taken as a single dose (or a short course for prevention), so this isn't an everyday juggling act. To be safe, just separate the two by a few hours. Baloxavir itself does ask you to avoid taking it with dairy, calcium, or other minerals, so spacing them out helps both products. Since these supplements aren't standardized, loop in your pharmacist or doctor before mixing them.

By severity: 1 major · 7 moderate · 9 minor.
From our pharmacist’s overview on the Baloxavir Marboxil page
“Activated charcoal is the supplement I would keep farthest from baloxavir marboxil (Xofluzanda), the single-dose flu antiviral, and it carries the only major rating among the 17 ingredients on file here. Charcoal adsorbs drugs in the gut, and because this whole flu treatment is one dose, a blunted dose has no follow-up pill to rescue it.”

About Thyroid Extract
What is Thyroid Extract?
- Hypothyroidism (underactive thyroid)
- Fatigue and low energy
- Weight management
- Sluggish metabolism
- General thyroid support
Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
