Here is the interaction we found between these two, with the evidence behind it.
Oral Drugs
Why this applies to Cabozantinib S-Malate
Cabozantinib S-Malate falls under Oral Drugs — the group Thyroid Extract interacts with.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the gist: this interaction is really about your thyroid extract being the one that gets affected, not your cabozantinib. The concern on file is that taking other oral pills alongside thyroid hormone can bind it up in your gut and lower how much thyroid hormone you actually absorb. So the worry is reduced thyroid effect, which could leave you feeling sluggish, cold, or tired.
To keep things simple, separate the two by taking your thyroid product on an empty stomach and waiting before other meds. One bigger picture note: cabozantinib (Cabometyx) is itself often used for a type of thyroid cancer, so please don't self-treat any thyroid issue with an unstandardized animal-gland supplement. Loop in your oncologist or pharmacist first so your real thyroid needs are handled properly.

From our pharmacist’s overview on the Cabozantinib S-Malate page
“Cabozantinib (Cabometyx) treats kidney, liver, and thyroid cancers, and because its dosing is calibrated carefully, its supplement list warrants genuine care. Of the 138 ingredients with a known interaction, the great majority involve the CYP3A4 enzyme that clears the drug, so an herb can push cabozantinib levels high enough to worsen…”

About Thyroid Extract
What is Thyroid Extract?
- Hypothyroidism (underactive thyroid)
- Fatigue and low energy
- Weight management
- Sluggish metabolism
- General thyroid support
Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
