Here's the gist for your Parkinson's medicine: this interaction is really about your thyroid extract, not so much the carbidopa/levodopa. Taking the two close together in your stomach can blunt how well the thyroid hormone gets absorbed, so the worry is the thyroid product working less, not your Sinemet behaving differently.

That said, it's smart to space them out. Thyroid hormone likes an empty stomach, while carbidopa/levodopa is its own juggling act with timing and protein. If they're competing in your gut, you may not get steady benefit from either, and with Parkinson's that can mean more wearing-off, stiffness, or unpredictable movement. Honestly, I'd skip self-treating thyroid issues with extract and let your doctor or pharmacist sort out the timing and any real thyroid testing.

About Levodopa and Carbidopa

What Is Levodopa and Carbidopa Used For?

Brand names: Sinemet CR, Sinemet, Rytary, Dhivy

Levodopa and carbidopa is used to treat Parkinson's disease, post-encephalitic parkinsonism (parkinsonism that develops after a brain infection), and symptomatic parkinsonism that can follow carbon monoxide or manganese poisoning. These indications are shared across the oral tablet, orally disintegrating tablet, extended-release tablet, and extended-release capsule formulations (including Sinemet, Rytary, Crexont, and generic products).

DUOPA — a carbidopa/levodopa suspension given through a tube directly into the small intestine — is specifically indicated for the treatment of motor fluctuations in patients with advanced Parkinson's disease. This is a product-specific indication and does not apply to the oral forms.
