Darunavir, Ritonavir falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other direction from what you might expect. It is not that your HIV medicine, darunavir with ritonavir, changes the supplement much. Instead, taking a thyroid extract at the same time as other pills and food can lower how well the thyroid hormone itself gets absorbed. Thyroid hormone is fussy and likes an empty stomach.

So if you swallow your darunavir/ritonavir, which is usually taken with food, right alongside a thyroid product, you may absorb less of the thyroid hormone. That can mean lingering low-thyroid symptoms like tiredness, feeling cold, or sluggishness. This is a minor, well-documented timing issue, easily fixed by spacing them apart. Please let your pharmacist or doctor set the schedule, and don't self-treat thyroid problems with unstandardized extracts.
