Ivacaftor, Lumacaftor falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other way from most interactions. It is your Orkambi (lumacaftor/ivacaftor) that can affect how well a thyroid supplement is absorbed, rather than the reverse. Thyroid hormones (T3 and T4) are picky about absorption, and taking them alongside food or other medicines can lower how much your body picks up. If your thyroid product is competing for stomach space with Orkambi, you might get less of the thyroid effect, which could leave thyroid symptoms (tiredness, feeling cold, sluggishness) less controlled.

The good news is this is rated minor and is mostly about timing. Spacing the two apart usually solves it. But thyroid extract products are not standardized, so please loop in your doctor or pharmacist before mixing them with Orkambi.

About Lumacaftor and Ivacaftor

What Is Lumacaftor and Ivacaftor Used For?

Brand name: Orkambi

Orkambi is approved to treat cystic fibrosis (CF) in patients age 1 year and older who have two copies of the F508del mutation in the CFTR gene — one on each chromosome. This is called being homozygous for F508del. If a patient's genetic type is unknown, a lab test approved by the FDA should confirm the mutation before starting treatment.

Orkambi has not been proven safe or effective for people with CF who have a different mutation pattern. It is only for the F508del/F508del genotype.
