Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the thing to keep in mind: this interaction runs in the other direction from what you might expect. Your Harvoni (ledipasvir/sofosbuvir) isn't the one being affected. Instead, taking it close together with thyroid extract could lower how well the thyroid hormone gets absorbed, since other oral pills and food can bind it up in your gut and blunt its effect.

The good news is this is rated minor and easy to manage. Thyroid hormone simply works best on an empty stomach, so a common approach is to take it first thing in the morning and space your Harvoni and breakfast a bit later. Your Harvoni's job of clearing the hepatitis C virus shouldn't be hurt here. Still, since over-the-counter thyroid extract is unpredictable, please loop in your pharmacist or doctor about proper timing and whether prescription thyroid medicine is the safer route.

About Ledipasvir and Sofosbuvir

What Is Ledipasvir and Sofosbuvir Used For?

Brand name: Harvoni

Ledipasvir and sofosbuvir treats chronic hepatitis C virus (HCV) infection in adults and pediatric patients 3 years of age and older. It is approved for HCV genotypes 1, 4, 5, and 6 in patients without cirrhosis or with compensated cirrhosis (mild-to-moderate liver scarring that the liver is still managing), and for genotype 1 with decompensated cirrhosis (advanced liver disease) when used together with ribavirin.

It is also approved for liver transplant recipients with HCV genotype 1 or 4 infection who do not have cirrhosis or have compensated cirrhosis, in combination with ribavirin. Both Harvoni and the generic ledipasvir and sofosbuvir tablets share these indications.
