Mexiletine falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern flips compared to most interactions. It isn't that mexiletine changes in your body, it's that mexiletine (an oral pill) is one of the things sharing space in your stomach with your thyroid extract. Anything you swallow alongside thyroid hormone can get in the way of how well that hormone is absorbed. So the practical worry is that taking these together could leave you absorbing less of your thyroid hormone, not that your heart medicine becomes stronger or weaker.

The simple fix is timing. Thyroid hormone works best on an empty stomach, taken by itself at least 30 to 60 minutes before other pills or food. Spacing your mexiletine away from it usually keeps both working as intended. Since thyroid extract doses aren't standardized, please confirm your plan with your pharmacist or doctor.

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
