Sevelamer falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the thing about sevelamer (Renagel): it works like a sponge in your gut, grabbing onto phosphorus from your food so it doesn't get absorbed. The trouble is, that same binding action isn't picky. If you take a thyroid extract (which contains real T4 and T3) at the same time, sevelamer can latch onto the thyroid hormone too and carry it out of your body before it gets absorbed. That means you may get less benefit from the thyroid product, even though sevelamer's own job isn't affected.

This is a well-documented, predictable interaction, though usually a mild one that comes down to timing. Spacing the two doses apart by several hours often solves it. Since these products work best on an empty stomach, please ask your pharmacist or doctor how to schedule them safely.

About Sevelamer

What Is Sevelamer Used For?

Brand name: Renagel

Sevelamer is used to control serum phosphorus (the level of phosphorus in your blood) in people with chronic kidney disease (CKD) who are on dialysis. High phosphorus is a serious problem for dialysis patients and needs to be managed with medication and diet.

Sevelamer carbonate tablets and Renvela® (sevelamer carbonate) oral suspension are approved for adults and children 6 years of age and older. Sevelamer hydrochloride tablets are indicated for adults with CKD on dialysis; their safety and effectiveness in CKD patients not on dialysis have not been studied.
