Thyroid Extract and Sofosbuvir, Velpatasvir, Voxilaprevir

Here is the interaction we found between these two, with the evidence behind it.

Minor

Oral Drugs

Why this applies to Sofosbuvir, Velpatasvir, Voxilaprevir

Sofosbuvir, Velpatasvir, Voxilaprevir falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.
Severity Minor Likelihood Likely Evidence A

Well-controlled human studies have demonstrated the likely existence of this interaction.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here the concern runs the other direction from what you might expect. Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) doesn't change how your thyroid extract works. Instead, it's your thyroid hormone that can be affected, since taking it too close to food or other pills can block how much your gut absorbs. This is a well-documented but generally minor issue, mostly about timing.

The practical fix is simple: take your thyroid product on an empty stomach, ideally well before your Vosevi dose and breakfast. If absorption drops, you might notice your old thyroid symptoms creeping back (tiredness, feeling cold, sluggishness). I'd really encourage you to work with your doctor on proper prescription thyroid treatment rather than over-the-counter extract, and let your pharmacist help you space everything out.

From our pharmacist’s overview on the Sofosbuvir, Velpatasvir, Voxilaprevir page

“Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) cures hepatitis C over a single short course, and that is exactly why I would be genuinely careful with supplements while taking it. Anything that drags its blood levels down can cost you the cure itself.”

About Thyroid Extract

What is Thyroid Extract?

Thyroid Extract
  • Hypothyroidism (underactive thyroid)
  • Fatigue and low energy
  • Weight management
  • Sluggish metabolism
  • General thyroid support

Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
