Tenapanor Hydrochloride falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the practical issue when you pair these two. Tenapanor (Ibsrela) stays mostly in your gut to do its job for IBS with constipation, and one of its main effects is looser, more frequent stools. Anything that speeds things through your intestines can also affect how well your body absorbs other things you swallow, including the thyroid hormones (T3 and T4) found in thyroid extract.

Because thyroid hormone needs good gut absorption to work, taking it close to a medicine that loosens stools may mean you absorb a bit less of it. This is a minor concern, and separating them by several hours usually helps. Please loop in your pharmacist or doctor, especially since over-the-counter thyroid extract is unstandardized and prescription thyroid medicine is the safer choice.

Tenapanor is approved for two distinct conditions, each under a separate brand. Ibsrela is used to treat irritable bowel syndrome with constipation (IBS-C) in adults. XPHOZAH is used to reduce high phosphorus levels (hyperphosphatemia) in adults with chronic kidney disease (CKD) who are on dialysis — specifically as an add-on when phosphate binders haven't worked well enough or aren't tolerated.
