Toremifene Citrate falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the thing to know about pairing thyroid extract with toremifene (Fareston): the concern flows in the other direction from what you might expect. It isn't that toremifene changes your thyroid supplement. Rather, taking another oral pill alongside thyroid hormone can get in the way of how well that thyroid hormone is absorbed. Thyroid hormones like to be taken on an empty stomach, and crowding them with other medicines can blunt their uptake.

This is a minor issue, and the simple fix is timing. If you take thyroid extract, give it some space (ideally on an empty stomach, with other pills like toremifene taken later). Toremifene itself works by blocking estrogen and isn't really affected here. Still, run your full medication list by your pharmacist or doctor so the timing is right for you.
