Why this applies to Treprostinil (IV)

Treprostinil (IV) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Tocotrienols interacts with.

Concomitant use of tocotrienols with anticoagulant or antiplatelet agents might increase the risk of bleeding. However, this has not been reported in humans.
Taking tocotrienols orally inhibits experimentally-induced platelet aggregation in humans. Theoretically tocotrienols might increase the risk of bleeding if taken with antiplatelet or anticoagulant drugs. However, tocotrienols 400-800 mg daily have been used with aspirin and/or clopidogrel for 1 year with no clear cumulative antiplatelet effects and no reports of bleeding.

Here's the thing to keep in mind with this pairing. Treprostinil, given through an IV for pulmonary hypertension, doesn't just open up blood vessels in the lungs, it also makes your platelets less likely to clump together. Tocotrienols, a form of vitamin E, appear to do something similar based on lab work in people, gently nudging platelets to stick less. Stack two things that both nudge in that direction and the theoretical worry is a little more bleeding or easier bruising.

I'll be honest though, this hasn't actually been seen in real patients, and studies using fairly high tocotrienol doses alongside other blood-affecting drugs didn't show clear extra bleeding. So this is a low-level, mostly-on-paper concern. Still, since you're on IV treprostinil, it's worth a quick chat with your doctor or pharmacist before adding the supplement.

What Is Treprostinil Injection Used For?

Brand name: Remodulin

Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries that supply the lungs. It works to reduce the symptoms that make exercise difficult. Clinical studies included patients with moderate-to-severe PAH symptoms (NYHA Class II–IV) due to idiopathic or heritable PAH, PAH related to congenital heart defects, and PAH related to connective tissue diseases.

It is also used for PAH patients who need to transition off epoprostenol — a related medication — to slow the worsening of their condition during that switch. The risks and benefits of the transition should be carefully discussed with a specialist before starting.
