Here's the gist with Maralixibat (Livmarli) and tragacanth. Tragacanth is a gel-forming fiber, and when you swallow it, that slippery gel can sit in your gut alongside anything else you've taken. In theory, it could grab onto an oral medicine and change how much gets absorbed, sometimes a little less, sometimes a little more. I want to be upfront: this is a theoretical concern based on how the gum behaves, not something proven with Maralixibat specifically.

Maralixibat works right in your intestine to block bile acid uptake, so anything that coats the gut lining could nudge how well it does its job. If you'd like to use tragacanth, a simple fix is to space it out, taking it about 30 to 60 minutes after your Maralixibat dose. Check with your pharmacist before combining them.

Tragacanth is a plant gum used mainly as a thickener and emulsifier in foods, cosmetics, and medicines, and traditionally to soothe the throat and gut. There is very little solid human research showing it treats any specific condition. It is generally considered safe in food amounts, but anyone with allergies or who is pregnant or breastfeeding should check with a pharmacist or doctor first.

What Is Maralixibat Used For?

Brand name: Livmarli

Livmarli (maralixibat) is used to treat cholestatic pruritus (intense itching caused by bile acid buildup in the body) in two rare liver conditions. It is approved for patients aged 3 months and older with Alagille syndrome (ALGS), and for patients aged 12 months and older with progressive familial intrahepatic cholestasis (PFIC).

There is an important limitation for PFIC: Livmarli is not recommended for a specific subgroup of PFIC type 2 patients who have certain gene variants (ABCB11 variants) that result in a non-functional or completely absent bile salt export pump (BSEP) protein.
