Zedoary and Acalabrutinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Zedoary interacts with.

Theoretically, zedoary might increase levels of drugs metabolized by CYP3A4.
In-vitro research shows that a methanol extract of zedoary strongly inhibits the CYP3A4 metabolism of the tyrosine kinase inhibitors lapatinib and sorafenib, and to a lesser extent, gefitinib.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Calquence (acalabrutinib). Your body clears this medicine largely through a liver enzyme called CYP3A4. In lab studies, zedoary appears to slow down that same enzyme. If that happened in a real person, acalabrutinib could clear more slowly and build up in the blood, which can mean stronger effects and a higher chance of side effects like easy bruising or bleeding, headaches, infections, low blood counts, or irregular heartbeat.

I'll be honest with you: this is only a theoretical concern based on test-tube work, not human studies, and a sprinkle of zedoary as a spice is unlikely to matter. Concentrated supplement doses are the bigger question mark. Since acalabrutinib is a serious cancer drug, please check with your oncologist or pharmacist before adding zedoary.

From our pharmacist’s overview on the Acalabrutinib page

“For anyone taking acalabrutinib (Calquence) for chronic lymphocytic leukemia or lymphoma, the supplement list below deserves genuine scrutiny. Of 144 ingredients on file, nearly all act on CYP3A4, the liver enzyme that clears the drug, or on P-glycoprotein, a transporter that moves it, so they can push its level too high or leave too l…”

About Acalabrutinib

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
