Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Zedoary interacts with.

Theoretically, zedoary might increase levels of drugs metabolized by CYP3A4.
In-vitro research shows that a methanol extract of zedoary strongly inhibits the CYP3A4 metabolism of the tyrosine kinase inhibitors lapatinib and sorafenib, and to a lesser extent, gefitinib.

Here's the thinking on this pair. Your body relies on a liver enzyme called CYP3A4 to break down and clear alpelisib (Piqray). In lab dish studies, zedoary appears to block that same enzyme. If that blocking happened in real life, alpelisib could clear more slowly and build up in your blood, which can mean stronger effects and more side effects. With alpelisib that often shows up as high blood sugar, diarrhea, rash, nausea, and mouth sores.

I want to be honest about the strength of this, though. It is theoretical, coming from test-tube work, not from people taking both together. So I wouldn't panic, but since alpelisib is a serious cancer medicine, please run any zedoary supplement past your oncologist or pharmacist first.

What Is Alpelisib Used For?

Brand name: Piqray

Piqray is used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and PIK3CA-mutated. It is for use after the cancer has progressed on or after an endocrine-based (hormone-blocking) treatment. An FDA-approved test must detect the mutation.

Vijoice is used for adults and children 2 years and older with severe PROS who need systemic (whole-body) therapy. This use has accelerated approval, which is based on response rate and duration of response. Continued approval may depend on confirmatory trials.
