Uva ursi can cause nausea, vomiting, diarrhea, and stomach upset; at very high doses (20 grams or more), it has caused serious effects including convulsions and delirium, and doses of 30 grams or more may be fatal. Buchu and cubeb lack strong safety data at medicinal doses.

Asparagus can trigger allergic reactions in people sensitive to onion and garlic family plants, and a combination product containing asparagus root caused constipation, abdominal pain, nausea, and gallbladder complaints in up to 50% of users in one trial. During pregnancy, juniper berry, buchu, bear berry, and cubeb should all be avoided—they are rated unsafe or likely unsafe because they may affect the uterus or lack safety data.

Asparagus as food is likely safe in pregnancy, but medicinal amounts haven't been studied enough. For breastfeeding, buchu is possibly safe; the others lack enough information, so they're best avoided.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.
