L. acidophilus is generally well tolerated in healthy adults and children. That said, since these are live organisms, there's a small theoretical risk they could cause infection in certain people — cases of bacteremia, sepsis, and other serious infections have been reported, though most happen in people where the bacteria may have moved from somewhere else in the body.

Some people taking L. acidophilus with other probiotics report mild gastrointestinal side effects like bloating, gas, abdominal discomfort, or diarrhea, but these are uncommon. One trial noted a couple of cases of rash and itching, though it's unclear whether those were due to the L. acidophilus or another ingredient.
