What’s inside
Full disclosure
AlphaCre HD is a 5-ingredient performance formula. It contains sodium and potassium as electrolytes, creatine monohydrate to support muscle strength and athletic performance, and beta-alanine for endurance work.
Phosphorus rounds out the mineral profile. The product also contains inactive ingredients — maltodextrin, citric acid, malic acid, natural and artificial flavoring, dipotassium phosphate, disodium phosphate, silicon dioxide, and sucralose — which serve as sweeteners, fillers, and binders.
Does it work?
Moderate evidence
Creatine monohydrate in this formula is possibly effective for muscle strength, athletic performance, and sarcopenia (age-related muscle loss). Beta-alanine is possibly effective for athletic performance and physical performance.
Sodium and potassium are essential minerals but aren't rated for sports performance in our data. We hold no effectiveness data for phosphorus in supplement use.
How safe is it?
Well-documented data
Creatine is generally well tolerated in healthy adults, though it commonly causes dehydration, diarrhea, muscle cramps, and water retention. Beta-alanine typically causes harmless tingling and flushing of the skin, usually starting on the scalp and spreading for about an hour.
Sodium in normal dietary amounts is fine, but too much is linked to high blood pressure and heart strain — the safety data advises against supplementing with sodium beyond what food provides. Potassium from a normal diet is appropriate, but supplements can cause dangerously high blood levels in some people, especially those with kidney disease.
The data advises avoiding creatine and beta-alanine during pregnancy and breastfeeding due to insufficient safety information.
Meds to double-check
Moderate interaction found
Before taking this product, check with your doctor or pharmacist if you use blood pressure medications (antihypertensives), lithium, corticosteroids, potassium-sparing diuretics, ACE inhibitors, or ARBs. These are all Moderate-severity interactions.
Also flag it if you take didanosine (for HIV), sodium phosphate laxatives, or tolvaptan.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This is a workout supplement aimed at athletes looking to build strength and boost performance. If you take blood pressure medications, lithium, potassium-sparing diuretics, ACE inhibitors, or ARBs, check with your doctor or pharmacist before using it.
If you have kidney disease, talk it over first — potassium supplements need extra caution. Pregnant or breastfeeding?

AlphaCre HD Berry Lemonade by 1st Phorm Drug Interactions
HelloPharmacist Interaction Report
AlphaCre HD by 1st Phorm contains sodium and potassium, both of which interact with medications.
The most serious concern is sodium's effect on blood pressure medications. High sodium intake can reduce how well antihypertensive drugs work, and you may need higher doses to control your blood pressure.
Read the full breakdown — every affected drug type, severity by severity
Sodium also interacts with lithium (used for bipolar disorder) — too much dietary sodium can lower lithium levels and reduce its effect, while cutting back on sodium can raise lithium levels to dangerous amounts. It similarly affects corticosteroid drugs, didanosine, sodium phosphate laxatives, and tolvaptan, all carrying a risk of dangerously high sodium levels in your blood (hypernatremia).
These interactions are all rated Moderate.
Potassium in this product poses a Moderate risk if you're taking potassium-sparing diuretics, ACE inhibitors, or ARBs — all commonly used heart and blood pressure medications. These drugs already reduce how much potassium your body excretes, and adding more potassium from a supplement can push levels too high and cause irregular heartbeat or other cardiac problems.
We could not check phosphorus for interactions. Altogether, these interactions span 220 individual medications.

For each ingredient in AlphaCre HD Berry Lemonade with known interactions, here are the types of medications they can affect. Open any type for the detail — or search your exact drug in the checker above.
Sodium
Antihypertensive Drugs
Theoretically, a high intake of dietary sodium might reduce the effectiveness of antihypertensive drugs.
High intake of dietary sodium can increase systolic and diastolic blood pressure. Also, high intake of sodium may necessitate increased use of antihypertensive medications to achieve blood pressure control in some patients, such as those with chronic kidney disease.
Corticosteroids
Concomitant use of mineralocorticoids and some glucocorticoids with sodium supplements might increase the risk of hypernatremia.
Mineralocorticoids and some glucocorticoids (corticosteroids) cause sodium retention. This effect is dose-related and depends on mineralocorticoid potency. It is most common with hydrocortisone, cortisone, and fludrocortisone, followed by prednisone and prednisolone.
Didanosine (Videx)
Concomitant use of didanosine with additional sodium from dietary or supplemental sources may increase the risk of hypernatremia.
Didanosine formulations contain a significant amount of sodium.
Lithium
Altering dietary intake of sodium might alter the levels and clinical effects of lithium.
High sodium intake can reduce plasma concentrations of lithium by increasing lithium excretion. Reducing sodium intake can significantly increase plasma concentrations of lithium and cause lithium toxicity in patients being treated with lithium carbonate. Stabilizing sodium intake is shown to reduce the percentage of patients with lithium level fluctuations above 0.8 mEq/L. Patients taking lithium should avoid significant alterations in their dietary intake of sodium.
Sodium Phosphates
Theoretically, concomitant use of sodium phosphate with sodium supplements might increase the risk of hypernatremia.
Use of high doses (> 45 mL in 24 hours) of sodium phosphate, such as those used for bowel cleansing before surgery, can lead to serious electrolyte disturbances, including hypernatremia. The risk of hypernatremia is highest in the elderly and people with other risk factors for electrolyte disturbances.
Sodium-Containing Drugs
Concomitant use of sodium-containing drugs with additional sodium from dietary or supplemental sources may increase the risk of hypernatremia and long-term sodium-related complications.
The Chronic Disease Risk Reduction (CDRR) intake level of 2.3 grams of sodium daily indicates the intake at which it is believed that chronic disease risk increases for the apparently healthy population. Some medications contain high quantities of sodium. When used in conjunction with sodium supplements or high-sodium diets, the CDRR may be exceeded. Additionally, concomitant use may increase the risk for hypernatremia; this risk is highest in the elderly and people with other risk factors for electrolyte disturbances.
Tolvaptan (Samsca)
Theoretically, concomitant use of tolvaptan with sodium might increase the risk of hypernatremia.
Tolvaptan is a vasopressin receptor 2 antagonist that is used to increase sodium levels in patients with hyponatremia. Patients taking tolvaptan should use caution with the use of sodium salts such as sodium chloride.
Potassium
Ace Inhibitors (Aceis)
Using ACEIs with high doses of potassium increases the risk of hyperkalemia.
ACEIs block the actions of the renin-angiotensin-aldosterone system and reduce potassium excretion. Concomitant use of these drugs with potassium supplements increases the risk of hyperkalemia. However, concomitant use of these drugs with moderate dietary potassium intake (about 3775-5200 mg daily) does not increase serum potassium levels.
Angiotensin Receptor Blockers (Arbs)
Using ARBs with high doses of potassium increases the risk of hyperkalemia.
ARBs block the actions of the renin-angiotensin-aldosterone system and reduce potassium excretion. Concomitant use of these drugs with potassium supplements increases the risk of hyperkalemia. However, concomitant use of these drugs with moderate dietary potassium intake (about 3775-5200 mg daily) does not increase serum potassium levels.
Potassium-Sparing Diuretics
Concomitant use increases the risk of hyperkalemia.
Using potassium-sparing diuretics with potassium supplements increases the risk of hyperkalemia.
