Berberine Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Berberine against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This liquid contains 6 ingredients, of which several are berberine-bearing herbs: barberry, goldenthread, lobelia, Berberis vulgaris, Berberis repens, and Oregon grape. Berberine is the active alkaloid responsible for most of this product's effects and interactions.
The inactive ingredients are alcohol and glycerin, which serve as preservatives and solvents in the liquid form.
Does it work?
Moderate evidence
Oregon grape is possibly effective for psoriasis. For other conditions—atopic dermatitis (eczema), gastroesophageal reflux disease (GERD), and peptic ulcers—the evidence we hold is insufficient to rate it.
Lobelia is rated possibly ineffective for smoking cessation. No effectiveness data are on file for barberry, goldenthread, Berberis vulgaris, or Berberis repens.
How safe is it?
Well-documented data
Lobelia carries the most serious safety concern: it can be toxic at higher doses (600 mg or more of the leaf), causing sweating, rapid heartbeat, seizures, low body temperature, dangerously low blood pressure, and potentially death. Even lower doses—as little as 50 mg—can cause nausea, vomiting, diarrhea, coughing, dizziness, tremors, and throat irritation.
Oregon grape applied to skin is generally well tolerated, but oral use is less studied. Human safety data for oral berberine-containing herbs overall are limited, and quality varies between products.
Pregnancy and breastfeeding are of concern: goldenthread should be avoided in pregnancy and while breastfeeding because berberine may harm the fetus or pass to infants. Lobelia is rated likely unsafe in both pregnancy and lactation.
Oregon grape is traditionally avoided in pregnancy, and berberine may pass to breastfeeding infants.
Meds to double-check
Moderate interaction found
Before taking this product, check with your pharmacist if you use any medications broken down by your liver's CYP3A4, CYP2D6, or CYP2C9 enzymes (ask if you're unsure). Also flag: blood thinners or antiplatelet drugs (increased bleeding risk), blood pressure medications (risk of too-low pressure), diabetes medications (risk of low blood sugar), sedating drugs (increased drowsiness), lithium (potential lithium buildup), or cyclosporine (increased levels).
Lobelia's diuretic properties and berberine's blood-thinning and blood-pressure-lowering effects add to these concerns.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
If you take any medications—especially for heart rhythm, blood sugar, blood pressure, blood clotting, depression, or immune suppression—talk with your pharmacist before using this product. The berberine content can significantly alter how your body processes many drugs.
Lobelia's toxicity risk and the product's pregnancy and breastfeeding warnings make this one to discuss with your healthcare provider if you're pregnant, planning to become pregnant, or nursing.

Berberine by Earth Friend Herb Co. Drug Interactions
HelloPharmacist Interaction Report
Berberine by Earth Friend Herb Co. has documented interactions with a substantial number of medications, primarily through its berberine content in several of the herbs.
The most serious concerns involve multiple drug-metabolizing enzymes in your liver (called cytochrome P450 enzymes) that this product may slow down, causing certain medications to build up to higher levels in your bloodstream.
Read the full breakdown — every affected drug type, severity by severity
Oregon grape and goldthread—both berberine-containing ingredients—can inhibit three major liver enzymes: CYP3A4, CYP2D6, and CYP2C9. This means drugs that depend on these enzymes to break down could accumulate.
Additionally, berberine may thin your blood (anticoagulant/antiplatelet effect), lower your blood sugar, reduce blood pressure, and enhance sedation—all Moderate-severity concerns when combined with relevant medications. Cyclosporine, an immunosuppressant, is specifically flagged because berberine slows its metabolism.
Lobelia adds a Moderate interaction with lithium, potentially raising lithium levels through diuretic effects.
Barberry, Berberis vulgaris, and Berberis repens could not be checked against our data. Run your exact medications through the search tool on this page before starting this product.

Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Goldenthread
Cyclosporine (Neoral, Sandimmune)
Berberine, a constituent of goldthread, can reduce metabolism of cyclosporine and increase serum levels. It might inhibit cytochrome P450 3A4 (CYP3A4), which metabolizes cyclosporine.
Cytochrome P450 3A4 (Cyp3A4) Substrates
There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.
