Boswellia serrata is possibly effective for osteoarthritis — that's the best evidence we have for it. For everything else on the label — hay fever, Alzheimer disease, asthma, and cluster headache — the evidence isn't established enough to rate it.
Frankincense's effectiveness for any of its listed uses (acne, aging skin, back pain, cancer-related fatigue, radiation dermatitis) is also insufficiently proven in the data we hold.
How safe is it?
Well-documented data
Boswellia serrata is generally well tolerated when taken by mouth short-term, though quality and dose vary widely between products. The most common side effects are abdominal pain, diarrhea, headache, heartburn, itching, and nausea.
Dizziness and vertigo have also been reported. A rare but serious risk is bezoar formation — a blockage in your digestive tract — especially if you take large amounts or chew and swallow the resin.
The safety data advises against use during pregnancy and while breastfeeding. Frankincense has insufficient safety information for pregnancy and breastfeeding, so avoid it in both cases.

Immunosuppressants
Theoretically, Boswellia serrata might alter the effects of immunosuppressive drugs.
Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.
