Boswellia Serrata + Budesonide, Formoterol
Budesonide, Formoterol brand name: Symbicort.
Budesonide, Formoterol falls into 3 drug categories that Boswellia Serrata can interact with; the most serious is rated moderate. Theoretically, Boswellia serrata might increase the levels of CYP2D6 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Boswellia Serrata interacts with whole categories of medication. Budesonide, Formoterol falls into 3 of those categories, so it can interact with Boswellia Serrata in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 3Why this applies to Budesonide, Formoterol
Budesonide, Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Symbicort (budesonide and formoterol) and Boswellia. The concern on file is that boswellic acids might block a liver enzyme called CYP2D6, which could let certain drugs build up. The catch is that this idea comes only from test-tube research, so it's theoretical, not something seen in real patients.
On top of that, budesonide and formoterol aren't mainly cleared by CYP2D6, and you inhale them, so very little reaches your bloodstream to begin with. That makes a meaningful interaction unlikely. Still, if you ever notice extra jitteriness, a racing or pounding heart, or shakiness, mention it to your pharmacist or doctor before changing anything.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES
2 of 3Budesonide, Formoterol also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2C19 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Symbicort (budesonide and formoterol) and Boswellia. In a test tube, Boswellia appears to block an enzyme called CYP2C19, and the worry is that this could let certain medications build up a bit. Since neither budesonide nor formoterol is a prodrug, more drug hanging around would mean slightly stronger effects, not weaker ones.
I'll be honest though, this is a very theoretical concern. It's based only on lab data, and Symbicort isn't mainly cleared by CYP2C19 anyway (budesonide leans heavily on CYP3A4). So I wouldn't lose sleep over it. If you ever notice extra jitteriness, a racing heart, or tremor, just mention it to your pharmacist or doctor.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
3 of 3Budesonide, Formoterol also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2C9 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Symbicort (budesonide plus formoterol) and Boswellia. In lab dish studies, boswellic acids appear to slow down a liver enzyme called CYP2C9. If that enzyme is blocked, certain medicines it processes can build up a bit higher than usual. Since neither of these is a prodrug, that would mean slightly stronger effects rather than weaker ones.
Honestly though, this is a theoretical concern only, based on test-tube work, not on real people. Most of Symbicort's dose works right in the lungs, so a big interaction is unlikely. Still, if you noticed more jitteriness, a racing heart, or shakiness, mention it. It's always smart to run any new supplement past your pharmacist or doctor first.
